Download and complete the online enrollment form to apply for our Comprehensive Healthcare plan covering Health Insurance, Personal Accident, Critical Illness, Major Medical and more. Enrollment forms can also be collected at your Credit Union. Completed enrollment forms can be submitted to your Credit Union or emailed to:email@example.com. You must be a Credit Union member to access plan options.

A standing order with your credit union will be required to facilitate the monthly premium payment.
Health Cards & Claims Cheques/Claims
Health Cards and Claims cheques will be dispatched to your respective Credit Union. Submission of claim forms should be done at your credit union.
